Individuals can either be referred by a medical practitioner or they can self-refer, once they have been discharged from hospital.
Quest is fully funded by the HSE, this FREE programme is available to those who qualify for the service.
To apply to Quest:
- Download the GP Health Report (have your GP or Consultant complete this)
- Download and complete the relevant application form (see below)
- Return to: quest@nln.ie
- Or post directly to the Quest office (address can be found on the form).
Galway clients can fill in this Application Form and return it to:
